How do I: Replace my lost ID…

We wish this was easier, but hopefully these steps will minimize the running around trying to find what to do next.

  1. Go to House Staff Office to get a letter certifying your employment.

  2. Take this letter to the bursar/cashier’s office to pay $25 and get a receipt.

    • Cashier’s office: if walking in from 210 St. entrance, walk straight, make a right to go down the hallway past the 210 café. The cashier office will be on your right. It is for patients, too, but go up to the booth with tellers and tell them what you’re there for and it should be quick.

  3. Take the receipt and your letter to the security office 3324 Rochambeau Avenue

    • ID cards are given out Monday, Wednesday and Friday, 8:30-noon, 1pm-4p

  4. You now need to activate the card. To do this, you need to send the chiefs a photo of teh front and back of your new ID so they can email idaccess@montefiore.org and einidaccess@montefiore.org to request your access be reactivated. You should have access within 24-48 hours of this request.

  5. You also need to reactivate your daily stipend. To do this, call the house staff office 718-920-2341. They will need the 5-digit number on the back of your card (the one circled above, not the EZ ID). This will be restored within minutes of their confirmation.

  6. If you use Montefiore parking, you will also need to go to the parking office to replace the parking chip (this will also cost $ which you can pay for there). Einstein parking will need to be reset at Einstein.
