Child abuse consults (updated 9/2022)

Currently, there are no child abuse consults; though there aren’t official consults Providers with suspicion of maltreatment of any kind are mandated to report that suspicion.   

Please fax the report to the Butler Center. See attached fax cover sheet below.

No child should leave any of our facilities if there is reason to believe that the child is in imminent risk. In an infant or young child with injuries that are suspicious for physical abuse, the patient should be admitted for thorough evaluation and the recommended work up for occult injury.  Please refer to the attached guideline and algorithm for suggested evaluation of possible child physical abuse.

How to make a report as a Mandated Reporter:

1. Call the New York State Central Register of Child Abuse & Maltreatment (SCR) hotline directly at 1(800) 635-1522. 2.

2. Complete reporting form DSS-2221A. This must be done by the provider making the call.

3. Immediately fax or mail the completed form to the ACS Borough Office where the child resides. Completed forms for Bronx reports can be faxed to: (718) 933-8832 or mailed to the Administration for Children’s Services (ACS) Applications 1200 Waters Place, Bronx, NY 10461

4. Please also fax a copy of the completed form DSS-2221A to the Butler Center for Children and Families. Use their Fax Cover Sheet
