IMDS Access
a) Go to the website https://apps.health.ny.gov/pub/usertop.html and create a user ID and password.
b) Email Elaine Taylor (from the House Staff Office) at etaylor@montefiore.org with the following info:
- User ID and Password. You can just send the email you receive from the HSC.
- Copy of your NYS Drivers' License or Passport
- Email address
Scrub Request Form - Complete the form and fax to 718-542-5893, or bring down to the office in the basement of Weiler. The Proximity Card # is the last 5 digits found on the back of your Montefiore ID following the long string of digits (or may just be 5 numbers on their own). It takes 2-3 days for your card to be activated for the scrub machine. You can put supervisor as Aileen Mendez, expiration 3 years from when you send.
