Social Pediatrics Rounds
Site Visits: Opportunities within SPR sessions to visit organizations within the community to learn about resources and advocacy opportunities in the Bronx
Policy Rounds: The goal of Policy Rounds is to identify and discuss individual, family, and community concerns at the local, state, national and/or international level that impact children’s health. This session is an opportunity for residents to discuss current events and the policies related to them, in the context of social medicine. The presenter should choose an article (newspaper, journal etc.) on a recent event and give a synopsis, focusing on any policy and/or legislation related to the topic. Presenters may choose a topic that is related to the theme of the SPR month. However, it is most important for the topic to be of interest to the facilitator, and related to a current event that impacts children’s health.
The learner should provide the following for discussion:
Background for discussion as it pertains to the current issue/event
Synopsis on policy/legislation on the topic including sharing of original article
General public's response to the event (if known)
Outside Speakers: Throughout the year, speakers from outside Social Pediatrics will present on a given topic within the month’s theme in SPR. These experts provide guidance and context for the challenges our patients face in clinic. Many of them are alumni of the Social Pediatrics program!
Social Pediatrics Clinical Rotations and Primary Care Didactics
LINCS (FTC during 2nd year): Each PGY-2 will be scheduled for LINCS clinic during one of their FTC blocks. They will attend the clinic on Tuesdays and Thursday mornings during that block.
School Health Rotation (FTC during 2nd year): Each PGY-2 will participate in the school clinic at New Settlement school every Wednesday during one FTC month. The purpose of the school health rotation is to 1) experience primary care outside of the CHCC setting, 2) expand our interaction with adolescent health care, and 3) become proficient in reproductive health care. Please email Dr. Castelnuovo one week prior to starting to see what time you should arrive. The New Settlements school is located at: 1501 Jerome Avenue, Bronx, NY, 10452.
Mobile Van Rotation (FTC during 3rd year): Each PGY-3 will participate in a Mobile Van experience during two of the FTC blocks. You will be assigned to one of the Mobile Van sites for one full day during one of your FTC months.
Child Protection Center Rotation (FTC during 3nd year): Each PGY-3 will be scheduled for CAC during one of their FTC months. Dr. Jamie Hoffman-Rosenthal is your contact person; please email/text her prior to the session to let her know you will be with her that day.
Social Work Rounds (Wednesdays as available, in CHCC Precepting Room): On Wednesday throughout the year, CHCC Social Work will join us before seeing patients to discuss cases and present information on patient advocacy and community resources.
Social Medicine Curriculum (Cross-track with Family Practice and Social Internal Medicine)
Immersion Month: PGY1s engage in a month-long curriculum with their colleagues in Family Medicine and Social Internal Medicine introducing topics in social medicine.
Social Medicine Rounds: Second and fourth Tuesdays of each month from 4:30-6:00 pm. Location can vary but usually it is held at RPSM (3rd floor conference room). The lecture series is dedicated to topics in Social Medicine. Residents at SPR that day are expected to attend.
DFSM Courses: Courses within DFSM are offered during 3 cycles of the year for those on full-time clinic. Topics include medical Spanish, epidemiology, and health systems. You may need to consider this schedule when you are making your block schedule.
CHAM Lectures and Didactic activities
You are expected to attend all CHAM lectures (8:30am and 12:15pm lectures, morning reports, grand rounds, professors rounds) while on all rotations excluding ED, PICU, NICU, and WBN. You are expected to attend morning report on full time clinic.
Social Pediatrics Evaluations
Individualized Learning Plans: Each resident should meet at least twice per year with their assigned faculty mentor to develop and review the Individualized Learning Plan (ILP). This is a formal way to focus on your goals and educational needs and to ensure that you are achieving the ACGME competencies. Dr. Braganza will also review your ILP at your biannual meeting.
Clinical Observations: As an important part of your clinical training, a preceptor will observe your visit with a patient twice during the year. She will contact you about scheduling the observations and provide feedback on your performance.
Psychosocial Direct Observation: At least twice during the year, Dr. White-Davis will “shadow” each resident through their afternoon clinic panel, with a focus on building therapeutic alliances and interviewing skills. This direct observation will be interactive and is meant to make Dr. White-Davis available to one resident at a time.
New Innovations: Your procedure log is on the New Innovations website. You may also download the app to your phone. You should always list a supervisor for the first five of any procedure.
Requirements for each year are:
PGY1- 75
PGY2- 150 (cumulative)
PGY3- 225 (cumulative)
Biannual Review: Twice a year you will meet with Dr. Braganza to review your evaluations from CHAM and CHCC, go over your ILP, and discuss your progress. This is an ACGME requirement. You are welcome to review your files anytime; just schedule an appointment through Deyanira Acevedo.
Resident Wellness Session (Currently Suspended)
Residents will have the opportunity for protected time to focus on enhancing mental, physical, and emotional well-being within their full-time clinic block. ReWe sessions will be held twice per year at 45 min-1 hour blocks facilitated by the Social Pediatrics Behavioral Science Faculty.
GOAL:
To promote psychological and physical well-being of residents
To promote skills in self-reflection and reflection-in-practice
LEARNING OBJECTIVES:
Residents will:
Learn various relaxation, mindfulness, and self-reflection strategies
Learn to manage and improve emotional well-being
Learn to improve ability to apply skills to patient care
Logistics:
Session length: 45 min- 1hour sessions. For PGY-3’s the session will be at 1:00pm prior to their scheduled Wednesday afternoon clinic during PICU, NICU, or Heme-Onc rotations. For PGY-2’s the session will be at 1:00p.m. on a Friday during PICU, NICU, or Heme-Onc rotations. For PGY-1’s the session will be at 1:00p.m. on either Wednesday or Friday. (Date of ReWe is subject to resident and faculty availability).
Each resident will have at least 2 sessions per year in the fall and winter/spring afternoon primary care clinic
CONTENT OF SESSIONS MAY INCLUDE:
Relaxation Skills: Diaphragmatic breathing, Progressive Muscle Relaxation, Guided Imagery, Mindfulness
Self-reflection Skills: NARC, narrative therapy strategies (e.g. use of metaphors, mirroring)
Behavior Change strategies: motivational interviewing (e.g. decisional balance)
