BIOPSYCHOSOCIAL CASE CONFERENCE

The Social Pediatrics Biopsychosocial Case Conference has two primary goals:
To increase residents’ knowledge of the interface of biological, psychological, familial and social issues presented by patients and families
To facilitate residents’ management of and reflection on challenging cases
Learning Objectives:
On completion of the social pediatrics biopsychosocial case conference, the learner will be able to demonstrate the following competencies:
An enhanced understanding of the doctor-patient relationship
Describing a broad conceptualization of health that includes the integration of psychosocial issues
Recognizing effective interventions for patient care
Description:
Residents will select a patient and family from their current caseload to present to peers and faculty. This case should highlight an interesting and/or challenging case within the current year of residency that they would like to reflect upon and discuss in-depth with others.
The resident should focus on the psychosocial aspect of the history, identifying a key characteristic of the history as a teaching point to expand upon. The resident is expected to provide education on a topic that is directly related to the patient’s experience as it pertains to the psychosocial history.
The goal of this discussion is to give residents the opportunity to learn from peers, faculty, and their own experiences with similar cases. Presentation of the case should include a brief description of the following:
Case Conference:
Presentation
A brief summary statement of the patient that includes the most important characteristics of the entire history that are relevant to the discussion
For example; do not include PSH, medications or allergies in summary statement unless pertinent
Psychosocial information
Brief description of key encounters in clinic (do not need to present entire history of visits from initial to present)
Use of a genogram (to be reviewed with preceptor prior to the week of the case conference)
Discussion
To facilitate the discussion of the case:
Choose a topic and formulate a few questions for discussion that highlight a chosen theme (identified in collaboration with psychosocial preceptor)
What are the medical issues influencing the problem?
What are the psychosocial issues influencing the problem?
Familial issues
Peer relationships
Socioeconomic issues
Community issues
Cultural issues, etc.
What are the patient and family strengths and vulnerabilities, and how do they contribute to the topic being discussed within the session?
What are some possible systems and resources (e.g. community organizations, benefits) involved in the case, level of access, and utilization by patient/family as well as provider?
To wrap up the discussion:
Describe “lessons learned” through your experience with this patient/family
Include reflections about your experience: in visits, with patients, with care provided, with system(s) involved, with transference/counter-transference issues, etc.
Facilitate evidence-based learning and discussion with relevant article:
Choose one (1) article to share with group during the presentation, highlighting most salient points (discuss which article may be most appropriate with preceptor)
Give brief explanation of reason for choosing article
(* Note: In some cases you may not have all aforementioned information. In such cases the presentation would focus on the information to be gathered.)
Social Pediatrics and/or Behavioral Science faculty will be available for assistance with preparation of all aspects of the presentation.
Social Pediatrics and/or Behavioral Science faculty will:
Meet with residents at least 2 weeks prior to presentation to identify appropriate case, goals and objectives
Train residents to further develop case formulation and conceptualization from a biopsychosocial perspective
Train residents to further develop skills in self-reflection and reflective practice through processing of experiential learning
Provide supplemental materials that may be useful to resident’s understanding of psychosocial factors relevant to case
Assist in facilitating discussion of case during presentation. (Note: Presentation format may vary depending on nature of case. [e.g. focus on doctor-patient relationship, counter-transference, medical procedure, etc.])
