A Guide to Neuro Checks for Pediatricians (6/27/17)

Performing a neuro check can give you a great deal of information in 5 minutes or less. The goal of a neuro check is to monitor for changes in mental status and the development of new neurological deficits.  This abbreviated checklist presumes a patient in an altered or obtunded state.

 Neuro Checklist:

□ Heart rate/Blood pressure/Respiration rate

□ Anterior Fontanelle (if age-appropriate)

□ Mental status: level of consciousness

□ Cranial nerves including pupils, +/- cranial nerve reflexes (corneals/VOR/gag), fundoscopic examination

□ Motor/sensory: Assessment of movement (spontaneous/withdrawal/posturing) and of symmetry.

□ Reflexes: Babinski (toes up or down)

□ Compare to prior and DOCUMENT YOUR EXAM

Other examinations may be necessary based on the presentation and concerns for your particular patient.
