 From Dr. Justin Kopa, previous Streetworks physician: Streetworks is one of the multiple mobile sites that the Children's Health Project goes to throughout the city.  The site itself is a drop-in center for youth (up to age 24) who are homeless or insufficiently housed. We work out of a mobile medical unit that parks about 2 blocks away from Streetworks on 126th and Adam Clayton Powell, right behind the Adam Clayton Powell State building.  If you walk to 126th and Adam Clayton Powell from either the D or the 2 train, you will see the big blue bus.  Our clinic session begins at 1:00 and goes until 5 or 5:30. We usually see between 3 and 7 patients in an afternoon.  The low volume gives us plenty of time to spend with patients, which I try to use to explore not only their acute medical concerns but also the broader context of their homelessness.  Because one of the challenges in working successfully in this community is building trust between the provider and the patients, most of what you'll be doing is shadowing. For patients that are comfortable with that, I encourage you to actively engage in the conversation as you feel comfortable.

Once the information has been entered into the Department of Education (DOE) Personnel Eligibility Tracking System (PETS) by HR, email instructions are sent by the DOE to the resident/fellow/learner on how to complete the process. There is a time limit for the fingerprinting, so it’s important that the resident/fellow/learner be on the lookout for the DOE email.

 This clearance should be to be done well in advance of the rotation.

 Monte School health will submit your name and information provided into the secure PETS Database.  Please remember that everyone must go to 65 Court Street in Brooklyn for fingerprinting.

The email address given will receive an email form.

You must bring the Fingerprint Referral Form and a valid government-issued photo ID (US Passport, State driver's license, etc.) to:

The HR Connect Walk-In Center is open every business day from 8:30 AM to 5:00 PM. The telephone number is 718-935-4000. The cost of fingerprinting is $135; you may pay by check, money order, or credit card (Note: cash and American Express cards are NOT accepted). Please be fingerprinted as soon as possible, but no later than 10 days after you receive this email and form. Please note that this process is required in order for your security clearance to proceed.
**Please remember that you may not rotate or provide services in school health until your program confirms in PETS your eligibility to start the rotation. During your Orientation to School Health ( if not before) instructions will be given for reimbursement. Please provide your original receipts for the reimbursement.
