Floor and Bed assignments are determined by the nursing staff in collaboration with the Logistics/Admitting office. Team assignments are then determined by Managing Senior, after a bed has been assigned. If there are questions or concerns about floor assignments that require physician input, nursing leadership should contact the chief residents, PHM attending “Managing Senior Back-up” (listed in amion), or Pat Hametz.

  • All teams (other than Heme/Onc team) have a maximum of 16 total patients, with maximum of 14 PHM patients

  • The following criteria will be used to assign a patient to a team (in this order of priority in general):

    • Primary subspecialty team

      • Adolescent, Endo  → Blue

      • cardiology, GI  → Silver

      • liver, renal, rheum  → Purple

      • post op ortho  → Pink

      • neuro, pulm, genetics  → Green

    • For PHM patients, consider geographic location, particularly for patients who are acutely ill (‘watchers’) or have the potential to be high acuity

      • CHAM 6  → Blue, Pink, Purple, Green

      • CHAM 8 → Green, Pink, Silver

      • CHAM 9  → Purple

    • Teams should not have patients on more than 2 floors, and no more than 4 teams per floor

    • surgical patients by subspecialty

    • number of patients per team (ideally within a range of 2 or 3 in general), taking into consideration patient acuity/complexity and team staffing
